1. Select Create a Service Request.
2. Enter the address of the issue.
3. Press Next.
4. Select the Department or type in the issue.
5. Describe the issue in the comment section.
6. Upload photos of the issue (if applicable).
7. Click Next.
8. Enter your contact information.
9. Click Create Request!
