Food Handler’s Cards

The Food Handler’s Card is required for individuals who distribute or prepare packages or raw foods to the public. Compliance with Food Handler’s cards reduces the incidence of food borne illnesses.

Testing includes TB (Tuberculosis) test. All patients getting a test must return within 48-72 hours for reading and to receive their card.

Food Handler’s cards and TB tests are provided by appointment only. M,Tu,W and F only– there is NO TESTING on Thursday.

FEE LIST

  • FOOD HANDLER’S CARD – $15.00
  • TB TEST – $10.00
  • TB TEST (non-resident) – $15.00
  • DUPLICATE HEALTH CARD – $15.00
  • DUPLICATE TB TEST RESULTS – $10.00
